The menu options at the bottom should contain all the presets you have in your Flash Renamer. If it is having trouble writing to the file, then they might not appear, I guess. Make sure the file is not write protected for some reason.

What happens if you remove the shell.dat file temporarily and start/close Flash Renamer? Will it re-create the file for you?
