Thank you for the pdf. Having looked at it here is my conclusion:
This is based on a scanned original book. The person who scanned it has scanned each page as an image, and then converted the text to "computer text" and printed this text on top of the original to improve the readability. In addition to this, the background image in the pdf is very low detail, and probably just there to make it look a bit more like the original.
My program only extracts bitmap images (= the backgrounds). That is why the (computer) text is not included in these images.
