First of all, in case you missed it, when you registered you got a serial number by email. When you start PDF Wiz you should see a "link" that says something like "enter code". Click on this and enter your serial number.
Now for your question:
PDF File: This is the file you want to extract images from.
Output folder: The extracted images will be saved in this folder.
Base name: Each extracted image will get this name plus a number.
