I've seen PDF creators do many strange things to images. In this case it seems like the program that created the pdf has stored one of the images internally in a distorted way, and then when the pdf is rendered to screen (ie displayd my Acrobat Reader) this is is compensated by the drawing instructions inside the pdf. Don't ask me why. (I've seen much worse, such as pdf writers that slice images in 100s of thin slices that are then displayed next to each other...)

My program only extracts the *raw* image data. So I'm afraid I can't do anything about this situation right now.
