I have tested Flash Renamer on Vista RC1, and it worked for me. But perhaps I did it wrong. The likely problem is that you need to run as an administrator since adding a context menu requires editing of a protected part of the registry.
Since you seem to like this feature, then I would like to invite you to test the new beta version of 5.2. There I have greatly enhanced this context menu! You can download it here:

Please test it and tell me if it works on Vista. If not, then try to run it as an administrator to see if that works. Tell me what happens. I will use this to see if I need to make any changes. Perhaps I need to detect if the contect menu could not be added and instruct the user to run as an administrator?

In Flash Renamer 5.11 after install there is no possibility to use the contextmenu's (right-click on a folder) in Windows Vista.
So each time you have to start the program from the startmenu en browse to the folder you want to rename.
In this way the tool lost his functionality.
